ĐÁNH GIÁ THỰC TRẠNG SAI SÓT THUỐC TRONG LIỆU PHÁP TIÊM TĨNH MẠCH CỦA ĐIỀU DƯỠNG TẠI MỘT BỆNH VIỆN CHUYÊN SÂU

Mục tiêu: Đánh giá thực trạng và phân tích các yếu tố liên quan đến sai sót thuốc trong liệu pháp tiêm tĩnh mạch của điều dưỡng theo tiêu chuẩn quốc tế JCI-PRIME.
Đối tượng và phương pháp: Nghiên cứu mô tả cắt ngang, quan sát trực tiếp 255 mũi tiêm tĩnh mạch qua đường truyền đặt sẵn của điều dưỡng, tại các khoa lâm sàng (nội, ngoại, hồi sức) một Bệnh viện chuyên sâu, từ tháng 3-6/2024. Công cụ thu thập là bảng kiểm xây dựng dựa trên tiêu chuẩn JCI-PRIME, nhấn mạnh vào tính khép kín và an toàn của quy trình.
Kết quả: Tỉ lệ sai sót chung ghi nhận ở mức cao (89,4%). Sai sót kĩ thuật chiếm ưu thế ở khâu chuẩn bị (66,3%) và ở khâu thực hành tiêm (61,9%). Nguyên nhân cốt lõi được xác định là bỏ qua bước thẩm định tính phù hợp của y lệnh và kiểm tra tương tác thuốc của điều dưỡng. Có 50,1% trường hợp mắc đồng thời 2 lỗi kĩ thuật. Không tìm thấy mối liên quan có ý nghĩa thống kê giữa đặc điểm nhân khẩu học, trình độ và thâm niên công tác của điều dưỡng với sai sót thuốc (p > 0,05).
Kết luận: Tỉ lệ sai sót thuốc cao phản ánh khoảng cách giữa thực hành lâm sàng hiện tại và tiêu chuẩn quốc tế khi chưa được đào tạo chuyên sâu. Bệnh viện cần cấp thiết chuẩn hóa quy trình, đặc biệt là tách biệt khâu chuẩn bị và nâng cao năng lực thẩm định thuốc cho điều dưỡng.
